【关键词】 面罩 【摘要】 目的 评价经双通道面罩对慢性阻塞性肺疾病患者进行无创正压机械通气的临床效果。方法 选择43例慢性阻塞性肺疾病(COPD)急性加重期合并Ⅱ型呼吸衰竭患者,随机使用双通道面罩(试验组22例)或使用单通道面罩(对照组21例)进行双水平气道正压无创机械通气4h,通过分析患者的呼吸频率、心率和动脉血气变化,比较两组机械通气的临床效果。结果 两组患者机械通气前后其呼吸频率、心率和动脉血二氧化碳分压(PaCO2)均显著降低(P<0.05或P<0.01),动脉血pH、氧分压(PaO2)均显著增高(P<0.05或P<0.01);双通道面罩组pH增高和PaCO2降低均显著优于单通道面罩组(P<0.05或P<0.01)。结论 经双通道面罩和经单通道面罩对COPD合并呼吸衰竭患者进行面罩机械通气(FMMV)均取得良好的临床效果,经双通道面罩对COPD合并呼吸衰竭患者进行FMMV可以减少CO2重复呼吸,更有利于降低PaCO2水平。 【关键词】 面罩;无创正压机械通气;肺疾病,阻塞性;呼吸衰竭 The effects of noninvasive positive pressure ventilation through two-way face mask for patients with chronic obstructive pulmonary disease 【Abstract】 Objective To evaluate the clinical effects of noninvasive positive pressure ventilation(NPPV) through two-way face mask for patients with chronic obstructive pulmonary disease(COPD).Methods 43 COPD patients with respiratory failure were divided equally at random into two groups:(1)two-way face mask (22 patients),(2)one-way face mask(as control,21 patients),both groups were treated with double channel noninvasive mechanical ventilator 4 hours, Arterial blood PH, PaO2 and PaCO2 were measured, The changes of respiratory frequency and Cardiac rate were recorded in both groups.Results The respiratory frequency、Cardiac rate and arterial blood PaCO2 improved prominently(P<0.05 or P<0.01) in and arterial blood PH、PaO2 increased in both groups(P<0.05 or P<0.01) before and after the treatment; the increasing of PH and decreasing of PaO2 of trial group are more significant than the control group(P<0.05 or P<0.01).Conclusion Both Noninvasive mechanical ventiltion through two-way and one-way face mask are benefit to the treatment of COPD patients with respiratory failure; NPPV through two-way face mask can reduce the rebreathing and decrease the PaCO2 more effectively. 【Key words】 face mask;noninvasive positive pressure ventilation;pulmonary disease;obstructive;respiratory failure 近年来,由于双水平气道正压通气(BiPAP)技术的普及,面罩机械通气(face mask mechanical ventilation,FMMV)已在临床得到广泛应用,尤其是应用于有慢性呼吸衰竭急性发作或早期呼吸衰竭的患者。然而,由于BiPAP呼吸机与患者通过一条管道吸气和呼气,有可能导致二氧化碳(CO2)重复呼吸,引起CO2排出量减少。陈荣昌[1]等将单通道面罩改为双通道面罩,经电脑检测,双通道面罩可基本消除CO2重复呼吸,有利于降低PaCO2水平。笔者经双通道面罩对22例慢性阻塞性肺疾病(COPD)急性加重期合并Ⅱ型呼吸衰竭患者进行FMMV,取得较满意的临床效果,现报告如下。 1 资料与方法 1.1 一般资料 COPD急性加重期合并Ⅱ型呼吸衰竭的住院患者43例,男31例,女12例,年龄52~78岁,平均(64.2±5.8)岁,诊断符合2002年中华医学会呼吸病学分会COPD诊治指南[2]。患者神志清醒,所有患者均无气胸、病理性低血压、严重心律失常、心功能不全、明显面部畸形、误吸倾向等无创通气的禁忌证。患者随机分为两组:试验组22例(使用双通道面罩),对照组21例(使用单通道面罩)。两组患者的基础情况见表1。
